Middleton Physio provides evidence-based physiotherapy assessment, treatment, and advice in-person and online. All care is delivered by HCPC-registered physiotherapists working under the Middleton Physio associate model and is covered by appropriate professional liability and public indemnity insurance.

Middleton Physio is operated by Calum Middleton, with associate physiotherapists working independently under the Middleton Physio brand. Each physiotherapist is responsible for their own clinical practice, professional registration, insurance, and all aspects of the care they provide.

3. Scope of Practice

The physiotherapist is trained to recognise when a condition falls outside their musculoskeletal scope of practice. Where appropriate, you may be referred to your GP or another healthcare professional for further investigation or management.

Middleton Physio does not provide emergency care. In an emergency, please contact 999 or attend your nearest emergency department.

4. Fees & Payment

Initial appointment (approx. 40 minutes): £65

Follow-up appointment (approx. 40 minutes): £65

Payment terms:

  • Online bookings: payment in full prior to appointment

  • In-person: payment at time of appointment (card or cash)

Prices may change with reasonable notice.

5. Patient Eligibility

Eligibility may vary depending on the treating physiotherapist.

Services are available to individuals aged 16 and over who are able to provide informed consent in accordance with UK law. Both physiotherapists within Middleton Physio routinely see patients aged 16 and over.

Patients under the age of 16 may be seen on a case-by-case basis where clinically appropriate. In these cases, consent must be provided by a parent or legal guardian, and a parent or guardian may be required to attend or remain present during the appointment where clinically or safeguarding appropriate.

6. Patient Responsibilities

You agree to:

  • Provide accurate and complete information about your condition

  • Complete any required forms honestly

  • Follow advice and exercise guidance to the best of your ability

7. Consent to Treatment

By attending or booking an appointment, you consent to assessment and treatment by the physiotherapist you are booked with.

You may:

  • Ask questions at any time

  • Decline or stop treatment at any point

  • Request a chaperone where appropriate

8. Clothing & Examination

Some assessments may require exposure of the area being treated. This will always be done with your consent.

Please wear appropriate clothing:

  • Upper body: vest or sports top

  • Lower body: shorts where possible

9. Cancellations & Missed Appointments

A minimum of 24 hours’ notice is required for cancellations.

Late cancellations or missed appointments may be charged in full, except in exceptional circumstances.

10. Confidentiality & Data Protection

Your information is handled in accordance with UK GDPR (2018) and professional confidentiality standards.

  • Records are stored securely in Cliniko (encrypted system with two-factor authentication)

  • No data is stored locally on personal devices

  • Information is only shared with your consent or where required by law

You have the right to request access to your records.

11. Record Retention

  • Adults: minimum 8 years after last contact

  • Under 18s: until age 25

12. Communication

You may receive communication via SMS, email, or WhatsApp relating to:

  • Appointments

  • Exercise programmes

  • Clinical follow-up

These platforms are not fully secure and should not be used for emergencies.

Marketing messages can be opted out of at any time.

13. Online Physiotherapy

Online physiotherapy is provided using platforms such as Microsoft Teams or WhatsApp.

You are responsible for:

  • Having a stable internet connection

  • Suitable device with camera and audio

Online consultations have limitations and may not fully replace in-person assessment. In some cases, an in-person review may be required.

14. Liability

Middleton Physio provides care in line with professional standards and reasonable skill and care.

We cannot guarantee specific outcomes. We are not liable for any loss or deterioration unless caused by proven negligence or as otherwise required by law.

15. Clinical Audit & Quality Improvement

Anonymised data may be used for audit and service improvement purposes.

No identifiable information is used without your consent.

16. Photography & Video

With your consent, photos or videos may be taken for clinical records or educational purposes.

Consent can be withdrawn at any time.

17. Complaints

If unresolved, you may escalate the complaint to the HCPC or other relevant regulatory bodies.

18. Termination of Services

We reserve the right to terminate treatment where:

  • It is clinically inappropriate or unsafe

  • There is non-compliance with treatment

  • There is abusive or inappropriate behaviour

  • Payment obligations are not met

Where appropriate, alternative services will be suggested.

19. Changes to Terms

These terms may be updated from time to time. The latest version will always be available and will apply from the date of publication.

Consent Statement

By booking or attending an appointment, you confirm that you:

  • Understand the risks and limitations of physiotherapy

  • Consent to assessment and treatment

  • Agree to the payment and cancellation terms

  • Consent to data processing as described above

  • Understand that outcomes cannot be guaranteed

  • May withdraw consent at any time during treatment
